Abstract
Respiratory medicine is a core discipline of clinical medicine that focuses on the prevention, diagnosis, treatment, and long-term management of diseases affecting the lungs, airways, pleura, and respiratory muscles. Respiratory disorders remain a major global health challenge, contributing significantly to morbidity, mortality, and healthcare utilization. This chapter provides a comprehensive overview of the principles of respiratory medicine, integrating respiratory anatomy and physiology with evidence-based approaches to clinical evaluation, diagnostic investigation, and therapeutic management. It discusses the assessment of respiratory symptoms such as cough, dyspnea, chest pain, wheezing, and hemoptysis, together with systematic physical examination and interpretation of clinical findings. Common respiratory diseases—including asthma, chronic obstructive pulmonary disease (COPD), pneumonia, tuberculosis, interstitial lung disease, bronchiectasis, pleural disorders, pulmonary embolism, sleep-related breathing disorders, and lung cancer—are reviewed with emphasis on their pathophysiology, clinical presentation, diagnosis, and current management guidelines. The chapter highlights the role of pulmonary function testing, arterial blood gas analysis, chest radiography, computed tomography (CT), bronchoscopy, microbiological investigations, and molecular diagnostic techniques in establishing accurate diagnoses. Evidence-based pharmacological therapies, including bronchodilators, corticosteroids, antimicrobial agents, biologics, anticoagulants, and oxygen therapy, are discussed alongside non-pharmacological interventions such as pulmonary rehabilitation, smoking cessation, vaccination, mechanical ventilation, and respiratory critical care. Emerging developments in respiratory medicine, including artificial intelligence-assisted imaging, telemedicine, precision medicine, and novel targeted therapies, are explored for their potential to improve diagnostic accuracy and patient outcomes. By integrating clinical expertise with current scientific evidence, healthcare professionals can deliver effective, patient-centered respiratory care.
