Abstract

Background: Infectious diseases remain a major global health challenge, contributing significantly to morbidity, mortality, and healthcare burden despite advances in vaccination, antimicrobial therapy, and public health interventions. Emerging and re-emerging infections, antimicrobial resistance (AMR), sepsis, and critical illness continue to threaten healthcare systems worldwide. Immunological mechanisms play a pivotal role in host defense, disease progression, and therapeutic response, while critical care medicine is essential for managing life-threatening infections and multisystem organ dysfunction.
Objective: This review aims to summarize the epidemiology, immunopathogenesis, clinical manifestations, diagnostic approaches, and evidence-based management of infectious diseases requiring immunological assessment and critical care, with emphasis on contemporary therapeutic strategies and multidisciplinary management.
Methods: A narrative review was conducted using current international clinical guidelines, systematic reviews, randomized controlled trials, and observational studies published in peer-reviewed medical literature. Evidence related to microbiological diagnostics, immunological investigations, molecular testing, antimicrobial therapy, infection prevention, sepsis management, organ support, and intensive care interventions was critically synthesized.
Results: Recent advances in molecular diagnostics, rapid pathogen identification, genomic surveillance, immunomodulatory therapies, antimicrobial stewardship, and precision critical care have significantly improved the diagnosis and management of infectious diseases. Early recognition of sepsis, prompt administration of appropriate antimicrobial therapy, hemodynamic optimization, ventilatory support, renal replacement therapy, and multidisciplinary intensive care management have contributed to improved survival. Preventive strategies, including vaccination, infection control measures, and surveillance programs, remain fundamental in reducing disease transmission and combating antimicrobial resistance.
Conclusion: Effective management of infectious diseases requires the integration of clinical expertise, immunological understanding, rapid diagnostic technologies, antimicrobial stewardship, and evidence-based critical care practices. Continued research in host-pathogen interactions, vaccine development, precision immunotherapy, artificial intelligence-assisted diagnostics, and global surveillance systems will be essential for improving patient outcomes and addressing the evolving challenges posed by infectious diseases and critical illness.
