Abstract
History taking, physical examination, and clinical documentation constitute the cornerstone of effective patient assessment and clinical decision-making. Despite rapid advancements in diagnostic technologies, a comprehensive medical history and systematic physical examination remain the most valuable tools for identifying patient problems, formulating differential diagnoses, and planning appropriate management. This chapter provides a structured overview of the principles, techniques, and clinical significance of patient evaluation, emphasizing the importance of establishing rapport, effective communication, and patient-centered interviewing. It discusses the components of a complete medical history, including presenting complaints, history of present illness, past medical and surgical history, medication use, allergies, family history, social history, and review of systems. Standard methods of physical examination, including inspection, palpation, percussion, and auscultation, are described with their relevance to detecting normal and abnormal clinical findings across different organ systems. The chapter also highlights the importance of maintaining accurate, comprehensive, and legally compliant clinical documentation using standardized formats such as SOAP (Subjective, Objective, Assessment, Plan) notes and electronic health records.
