Abstract
Diagnostic medicine plays a pivotal role in modern healthcare by facilitating accurate disease identification, monitoring treatment response, and guiding clinical decision-making. Laboratory investigations, when interpreted alongside a detailed clinical history and physical examination, provide objective evidence that supports diagnosis, prognosis, and therapeutic planning. This chapter presents the fundamental principles of diagnostic medicine, emphasizing the appropriate selection, interpretation, and clinical application of laboratory tests across a broad spectrum of medical conditions. It reviews commonly used hematological, biochemical, microbiological, immunological, molecular, and pathological investigations, highlighting their diagnostic utility, indications, limitations, and quality assurance requirements. The chapter also discusses the phases of laboratory testing, including pre-analytical, analytical, and post-analytical processes, and examines common sources of error that may influence test accuracy and reliability. Special attention is given to evidence-based test utilization, reference ranges, diagnostic sensitivity and specificity, predictive values, likelihood ratios, and the interpretation of laboratory findings within the clinical context. Advances in diagnostic technologies, including molecular diagnostics, polymerase chain reaction (PCR), next-generation sequencing, point-of-care testing, biomarkers, and artificial intelligence-assisted diagnostic systems, are explored for their growing impact on personalized and precision medicine. Ethical considerations, patient safety, cost-effectiveness, and appropriate stewardship of diagnostic resources are also addressed to promote rational investigation practices. Through the integration of laboratory data with clinical reasoning, healthcare professionals can improve diagnostic accuracy, reduce unnecessary testing, and optimize patient outcomes.
